1. Eligibility Requirements

• Must be a U.S.-based retail, restaurant, or service business
• 90% or more of transactions must be face-to-face
• Merchant must submit:
  • Cobalt POS Program Qualification Form
  • A recent credit card processing statement
• Must not be an existing Cobalt merchant
• Must not have previously received a POS system under this program

2. Program Terms

• Upon approval, the merchant will receive use of a Clover POS system with no upfront hardware cost
• The POS system remains the property of Cobalt Payments, Inc.
• Merchant must actively process payments exclusively through Cobalt Payments for the duration of use
• If the merchant stops processing with Cobalt, the POS system must be returned within 10 business days or be subject to equipment fees per the merchant agreement
• Hardware is provided as part of a bundled merchant services program and may be tied to minimum processing requirements

3. General Conditions

• Cobalt reserves the right to approve or decline applications at its sole discretion
• Offer valid while supplies last and may be modified or discontinued without notice
• Merchant is responsible for any applicable monthly software fees or third-party app subscriptions not included in the program

4. Tax & Reporting

• As the hardware is not being transferred in ownership, no 1099-MISC is issued unless other bonuses or payments are made
